Applying for Medi-Cal
How do I apply for Medi-Cal?
To apply for Medi-Cal you need to fill out an application and mail it in or, go to your local Department of Human Services and fill out an application there.
Print Out an Application
You can download the Medi-Cal application yourself and then print each of the linked documents. This application is available in other languages.
Request an Application by Phone
You can call (415) 863-9892 and ask to have a Medi-Cal application mailed to you (this number is also the place to call with questions about the application).
Apply in Person
On Monday through Friday from 8:00 a.m. to 5:00 p.m., you can go to your local Department of Human Services office and fill out (or pick up) an application.
